Overview

- Provide expert social work services to tenants, focusing on case management and community engagement.
- Collaborate with multidisciplinary teams to address complex resident concerns and advocate for their well-being.
- Engage with families and external agencies to ensure comprehensive support for residents.
- Coordinate community center events promoting mental and physical health, social skills, and technology education.
- Conduct apartment visits and liaise with hospitals to provide tailored assistance.
- Maintain detailed records and provide regular updates to the Resident Relations team.
- Utilize provided office equipment to perform onsite duties effectively.
Key Responsibilities & Duties
- Manage cases involving residents with mental disabilities, emotional disturbances, and domestic challenges.
- Provide referrals to mental health services, cleaning companies, and other agencies as needed.
- Lead discussions and events at the Community Center to promote health and social skills.
- Support Community Center operations and collaborate with the Community Center Manager.
- Advocate for residents by liaising with Resident Relations and external organizations.
- Conduct apartment visits to assess and address resident needs.
- Communicate with families and hospitals to coordinate care and services.
- Prepare weekly and monthly statistical reports for the Resident Relations team.
Job Requirements
- Master of Social Work degree and LMSW certification required.
- Minimum of 2 years of field experience in social work.
- Proficiency in computer applications such as Outlook, Asana, Word, and Teams.
- Strong connections to external agencies and resources.
- Excellent written and verbal communication skills.
- Ability to perform onsite duties effectively using provided office equipment.
- Experience in case management and advocacy for diverse populations.
