Job Description

Overview

  • Provide case management services to support housing stability and reduce social isolation for residents.
  • Conduct intakes and screen clients to assess presenting problems and needs.
  • Assist clients in accessing benefits, entitlements, and services through referrals and advocacy.
  • Develop and implement individualized care plans for clients based on their needs.
  • Maintain accurate case notes and client data using program data systems.
  • Provide supportive counseling and arrange translation services for clients as needed.
  • Prepare and maintain required documentation, reports, and statistics.
  • Participate actively in supervision and staff meetings to exchange information.
  • Adhere to organizational policies and strive for excellence in all job aspects.

Key Responsibilities & Duties

  • Conduct client intake and assessments to determine social, emotional, and environmental status.
  • Provide accurate information on benefits, entitlements, and case assistance.
  • Advocate and intervene with agencies on behalf of clients to secure services.
  • Develop and follow up on care plans tailored to individual client needs.
  • Maintain comprehensive case files and documentation for program compliance.
  • Compile statistics and prepare reports as required by the organization.
  • Provide supportive counseling and arrange necessary services for clients.
  • Supervise social work students and coordinate translation services when needed.
  • Participate in staff meetings and supervision to ensure effective communication.

Job Requirements

  • Bachelor’s Degree in a relevant field is required.
  • Minimum of one year of related experience or training.
  • Proficiency in Microsoft Office Suite and other relevant technology.
  • Excellent communication, listening, and customer service skills.
  • Ability to multi-task and work independently with attention to detail.
  • Bilingual Spanish skills are a plus but not required.
  • Commitment to treating clients with respect and dignity.
  • Adherence to organizational policies regarding absenteeism and appearance.
