Job Description

Overview

  • Provide case management services to support housing stability and reduce social isolation for residents.
  • Conduct client intakes, screenings, and assessments to address presenting problems effectively.
  • Assist clients in accessing benefits, entitlements, and programs through referrals and advocacy.
  • Develop and implement care plans tailored to individual client needs and circumstances.
  • Maintain accurate and complete documentation in the program’s data system.
  • Provide supportive counseling and arrange translation services as necessary.
  • Supervise social work students and participate actively in staff meetings.
  • Adhere to organizational policies and strive for excellence in interpersonal relationships.

Key Responsibilities & Duties

  • Conduct client intakes and screenings to identify presenting problems and appropriate interventions.
  • Provide accurate information and assistance with accessing benefits and entitlements.
  • Develop and implement individualized care plans for clients, ensuring follow-up and ongoing support.
  • Maintain detailed case notes and client data in the program’s data system.
  • Prepare required documentation, compile statistics, and generate reports as needed.
  • Provide supportive counseling and arrange for necessary services, including translations.
  • Supervise social work students and exchange information with colleagues promptly.
  • Participate in supervision and staff meetings to ensure effective service delivery.

Job Requirements

  • Bachelor’s Degree required, with one year of related experience or training.
  • Proficiency in Microsoft Office Suite and other relevant technology tools.
  • Excellent communication, listening, and customer service skills.
  • Ability to multi-task, work independently, and maintain attention to detail.
  • Bilingual Spanish skills are a plus but not required.
  • Commitment to treating clients with respect and dignity.
  • Adherence to organizational policies regarding absenteeism and appearance.
