Job Description

Overview

  • Provide case management services to support housing stability and reduce social isolation for residents in a community housing setting.
  • Conduct client intakes, screenings, and assessments to identify needs and develop tailored care plans.
  • Assist clients in accessing benefits, entitlements, and services through counseling, referrals, and advocacy efforts.
  • Maintain accurate and complete case notes, client data, and service delivery records using program data systems.
  • Provide supportive counseling and arrange translation services to ensure effective communication with clients.
  • Collaborate with colleagues and participate in staff meetings to exchange information and improve service delivery.
  • Supervise social work students and contribute to their professional development as needed.
  • Adhere to organizational policies and maintain professional standards in all aspects of work.

Key Responsibilities & Duties

  • Conduct client intake and screening to identify presenting issues and develop appropriate interventions.
  • Provide information on benefits, entitlements, and services, assisting clients with applications and referrals.
  • Develop and implement individualized care plans, ensuring ongoing support and follow-up.
  • Maintain accurate case files and documentation, compiling statistics and preparing reports as required.
  • Utilize program data systems to monitor client progress and service delivery.
  • Provide supportive counseling to clients, addressing social, emotional, and environmental needs.
  • Arrange translation services to ensure effective communication with clients from diverse backgrounds.
  • Participate in supervision and staff meetings to enhance service quality and team collaboration.
  • Perform additional duties as required to support organizational goals and client needs.

Job Requirements

  • Bachelor's degree required, preferably in social work, psychology, or related field.
  • Minimum of one year of related experience or training in case management.
  • Proficiency in Microsoft Office Suite and other relevant technologies.
  • Strong communication and listening skills with a focus on client respect and dignity.
  • Ability to multitask and work independently with attention to detail.
  • Bilingual Spanish skills are a plus for effective client communication.
  • Commitment to organizational policies and professional standards.
  • Experience in supervising social work students is advantageous.
  • Demonstrated ability to provide culturally competent and empathetic services.
