Registered Nurse

Responsibilities: 

• Complete of initial enrollment assessments and reassessments with focus on clinical, psychological, and social needs of the prospects as well as existing members utilizing Uniform Assessment System for (UAS-NY) in accordance with DOH approved policies
• Conducts a Uniform Assessment System for current members and potential members, appropriately enrolling (or not taking under care,) in the member’s home or Nursing Home within DOH regulatory timeframes and unit protocols with an error rate of less than 5%.
• Explains program, rules and benefits of the plan in an easy-to-understand language
• Reviews Member handbook, describes provider benefits and responsibilities, coordination of all covered services and Medicare benefits as applicable
• Conducts functional, psychosocial and environmental safety assessments
• Determines eligibility of enrollees, Establishes safe plan of care, Communicates plan of care to member, family, caregivers, healthcare professionals

Requirements:

• RN in the State of NY
• 2-3 years of UAS experience
• Ability to work independently
