Your Impact as Director of Case Management
Reporting directly to the Chief Financial Officer, you'll oversee the clinical, operational, and financial aspects of our Case Management and Social Work departments. This leadership role involves daily collaboration with patients, physicians, and multidisciplinary teams to ensure optimal care coordination and utilization management.
Key Responsibilities:
- Lead and mentor a team of case management and social work professionals
- Ensure regulatory compliance with CMS, Title 22, and Joint Commission standards
- Collaborate with clinical staff to optimize patient flow and discharge planning
- Develop and implement utilization management protocols
- Provide clinical oversight for complex patient cases
- Partner with leadership on strategic initiatives and quality improvement
What We're Looking For
Education & Experience:
- Bachelor's degree in healthcare-related field (Master's preferred)
- 5-7 years clinical experience in Case Management/Utilization Review
- Minimum 3 years leadership experience in Case Management/Utilization Review
- Acute care experience strongly preferred
Required Credentials:
- Current California RN license or Licensed Clinical Social Worker (LCSW)
- BLS/CPR certification (can obtain within 90 days)
- CCM or ACM certification preferred
