Job Description
Position Summary
The Collaborative Care Provider delivers direct depression care management and clinical services to patients with depression in assigned practices. Reporting to the Regional Director, this role involves working collaboratively with the healthcare team across the organization, utilizing independent judgment in delivering care and services. Psychiatric consultation is also a key component of this position, which requires advanced educational training specific to the duties detailed.
Essential Job Duties
- Provide collaborative care services, including direct mental health services, coordination of care for patients in primary care settings, and telephonic outreach and management of assigned cases.
- Complete patient care documentation promptly, ensuring all electronic encounters are closed within 24 hours.
- Refer patients for long-term counseling and other necessary services (e.g., substance abuse treatment, legal services, housing assistance).
- Utilize PHQ-9 and other standardized tools in patient assessments, adhering to agency guidelines for behavioral health diagnoses. Promote behavioral health workflow in assigned centers.
- Participate in department and organizational Continuous Quality Improvement (CQI) projects as required.
- Manage caseload effectively, using reporting tools for self-management.
- Actively participate in clinical supervision.
Key Knowledge, Skills, and Abilities
- Excellent knowledge of behavioral medicine and evidence-based treatments for depression, including primary care-based treatments.
- Ability to conduct brief patient contacts and make quick, accurate clinical assessments of mental and behavioral conditions.
- Comfort with the pace of primary care, working with an interdisciplinary team, and strong communication skills.
- Good knowledge of psychopharmacology.
- Ability to design and implement clinical pathways and protocols for treating selected chronic conditions.
- Technical knowledge of medical social work and relevant community resources, with the ability to develop linkages with community organizations.
- Thorough understanding of the social work profession and its standards.
- Ability to make independent judgments in delivering high standards of social work care and services.
- Strong organizational, interpersonal, oral, and written communication skills, with the ability to handle multiple assignments simultaneously.
