We’re partnering with a respected and high-performing hospital on the North Florida coast to identify an experienced, mission-driven nurse or social work leader for a Director of Case Management opportunity. 💡 The Impact You’ll Make

As the Director of Case Management, you’ll oversee the full spectrum of care coordination and discharge planning within a dynamic acute care environment. Your leadership will shape processes that drive high-quality outcomes, resource efficiency, and seamless transitions for patients and families.

You’ll be responsible for:

Leading and mentoring a high-performing interdisciplinary case management team

Driving quality improvements across utilization review, care transitions, and documentation

Collaborating closely with hospital leadership, providers, and external agencies

Overseeing departmental operations, staffing, onboarding, and compliance

Implementing performance improvement initiatives and sustainability strategies

Ensuring alignment with organizational goals and regulatory standards

✅ What You Bring to the Table

🎓 Bachelor’s degree required; Master’s in Nursing, Health Admin, or Business preferred

🩺 RN license, LCSW, or LMSW required

💼 3+ years of acute care case management experience preferred

📈 2+ years in a leadership role within case management strongly preferred

🤝 Exceptional communication, collaboration, and strategic planning skills

🔍 Proven ability to manage change, lead teams, and drive measurable outcomes
