Job Description

A Southern California Medical Center is seeking an experienced and dynamic Nurse Manager of Case Management to lead a high-performing team dedicated to ensuring effective patient care coordination and timely discharges. This vital leadership role requires a confident and proactive Registered Nurse with a strong foundation in acute care case management and a commitment to excellence in patient outcomes and compliance.

As the Nurse Manager, you will report directly to the Director of Case Management & Social Services and oversee a team of RN Case Managers and Case Management Assistants (15–18 FTEs). You’ll be responsible for ensuring patient progression through the continuum of care, identifying and resolving barriers to discharge, and guiding the department in accordance with hospital policies, regulatory standards, and patient needs.

Key Responsibilities:

  • Manage daily department operations, including staffing, attendance, rounding, and workload distribution
  • Support and guide timely, safe, and effective discharge planning
  • Collaborate with physicians, payers, and post-acute providers to ensure smooth care transitions
  • Assist with direct admits and patient throughput to reduce length of stay and optimize outcomes
  • Identify gaps in care progression and resolve delays through escalation and interdisciplinary coordination
  • Educate and hold staff accountable to internal processes and standards for care coordination

Qualifications:

  • Bachelor’s degree in Nursing (BSN) required; Master’s (MSN) preferred
  • Current California Registered Nurse license (or pending)
  • Minimum 3–5 years of acute hospital case management and/or nursing leadership experience
  • Expert knowledge of InterQual and Milliman (M&R) criteria
  • Strong understanding of hospital reimbursement across Medicare, Medi-Cal, Managed Care, and private insurers
  • Proven ability to manage complex discharge planning and lead difficult conversations with patients and families
  • Demonstrated critical thinking skills in high-stakes care transitions and care planning scenarios
