Job Description
Job Responsibilities:
- The Clinical Manager provides oversight during all operational hours of all patient care services and personnel.
- Oversees an interdisciplinary team which manages an active patient census of approximately 200 patients.
- Oversees the clinical integrity of practice of the field staff and independent contractors.
- Oversees the Assessment Planning and Coordination (APC) activities.
- Collaborates with QAPI and Education team to ensure staff, per diem and independent contractors have the tools to perform their roles, supporting individual growth, OASIS integrity and strong documentation skills.
- Evaluates clinical competency in area of practice through direct observation (to include OASIS walks), skills assessment, and performance appraisals.
- Oversees compliance with timely submission of all documentation.
- Oversees the assignment of care, accuracy of the census and flow of documentation through support of team members.
- Participates with Management Staff in the design and revision of policies and procedures re: Agency systems, structure, and processes.
- Counsels and takes appropriate disciplinary action when departmental policy or professional standards are violated.
- Participates in Quality Assurance Performance Improvement (QAPI) activities of the Agency, collaborating with the Director of QAPI and Education.

Job Qualifications:
- License to practice as a Registered Professional Nurse in New York State.
- Baccalaureate degree in Nursing field preferred.
- Two years’ experience in home care and a Baccalaureate degree or the following combination of education, experience, and/or training: Four years’ experience in home care, and six credit hours, or the equivalent of education/training in public health and principles of management. Familiar with home care and its implementation. Skilled in the techniques of therapeutic communication. Demonstrates leadership abilities.
