Key Responsibilities & Duties

• Provide visiting home health services, including admissions, re-certifications, and discharges.
• Develop physician care plans in coordination with healthcare professionals.
• Ensure timely documentation using tablets within 24-48 hours.
• Conduct OASIS D1 assessments for patient admissions and transitions.
• Maintain compliance with Department of Health and government regulations.
• Promote patient education on prevention and healthy habits.
• Support the agency’s mission to provide culturally diverse and high-quality care.
• Report daily to the Clinical Manager and manage an assigned caseload.

Job Requirements

• Valid Nursing license and proven experience as a registered nurse.
• Associate of Applied Science (AAS) degree in nursing required.
• Experience in acute care, hospital settings, or home care preferred.
• Proficiency in comprehensive OASIS assessments and documentation.
• Pediatric experience and ability to drive with a personal vehicle.
• Familiarity with Homecare Homebase (HCHB) software is advantageous.
• Strong communication skills and ability to work independently.
