Key Responsibilities & Duties
- Provide visiting home health services, including admissions, re-certifications, and discharges.
- Develop physician care plans in coordination with healthcare professionals.
- Ensure timely documentation using tablets within 24-48 hours.
- Conduct OASIS D1 assessments for patient admissions and transitions.
- Maintain compliance with Department of Health and government regulations.
- Promote patient education on prevention and healthy habits.
- Support the agency’s mission to provide culturally diverse and high-quality care.
- Report daily to the Clinical Manager and manage an assigned caseload.
Job Requirements
- Valid Nursing license and proven experience as a registered nurse.
- Associate of Applied Science (AAS) degree in nursing required.
- Experience in acute care, hospital settings, or home care preferred.
- Proficiency in comprehensive OASIS assessments and documentation.
- Pediatric experience and ability to drive with a personal vehicle.
- Familiarity with Homecare Homebase (HCHB) software is advantageous.
- Strong communication skills and ability to work independently.
