Overview

  • Join a dedicated healthcare team as a Licensed Practical Nurse (LPN) in a contract role.
  • Provide exceptional care to residents in a long-term care setting.
  • Work on-site at a facility located in New York City.
  • Perform duties during day shifts, 7 AM to 3 PM, with rotating weekends.
  • Collaborate with a multidisciplinary team to ensure patient safety and satisfaction.

Key Responsibilities & Duties

  • Administer medications and treatments as prescribed by physicians.
  • Monitor patient health and report changes to the Registered Nurse or Clinical Care Manager.
  • Maintain accurate and detailed patient records and documentation.
  • Provide direct care and support to residents, ensuring their comfort and well-being.
  • Assist in planning and implementing individualized care plans.
  • Participate in staff meetings and training sessions to enhance professional skills.
  • Ensure compliance with healthcare regulations and facility policies.
  • Support the team in emergency situations and provide timely interventions.

Job Requirements

  • Valid New York State LPN license is required.
  • Minimum of 1 year of experience in long-term care or geriatric settings.
  • Graduate of an accredited practical nursing program.
  • BCLS and IV certification are mandatory for this role.
  • Commitment to working every other weekend is essential.
  • Strong communication and organizational skills to manage patient care effectively.
  • Ability to work collaboratively in a team-oriented healthcare environment.
