Overview
- Serve as the primary liaison between members and the plan, ensuring seamless communication and coordination of services.
- Review and implement person-centered service plans, addressing members' unique needs and preferences.
- Coordinate covered and non-covered benefits, ensuring members receive comprehensive care and support.
- Maintain regular monthly contact with members to assess needs and provide follow-up support.
- Document all interactions and updates in members' charts in a timely and thorough manner.
- Participate in weekly team meetings to discuss member care and share insights.
- Collaborate with family members and vendor agencies to ensure coordinated care delivery.
- Adapt to additional duties as required to support the role's objectives.

Job Requirements
- Bachelor of Arts (BA) degree required, preferably in a relevant field.
- Minimum of 1 year of experience in case management or related roles.
- Proficiency in member assessment and clinical review procedures.
- Strong communication skills to interact effectively with members and stakeholders.
- Ability to interpret documents and apply information to member care.
- Experience in documenting and maintaining accurate records of member interactions.
