Key Responsibilities & Duties
- Provide visiting home health services, including patient admissions, re-certifications, and discharges.
- Develop and coordinate Physician Plans of Care with healthcare professionals.
- Document patient care using a tablet within the required timeframe.
- Utilize OASIS D1 assessments for admissions, re-certifications, transfers, and discharges.
- Ensure compliance with Department of Health regulations and agency standards.
- Support the agency’s mission by delivering culturally competent and high-quality care.
- Collaborate with the Clinical Manager and healthcare team to achieve patient recovery goals.
Job Requirements
- Valid Nursing license and proven experience as a registered nurse.
- Associate of Applied Science (AAS) degree in Nursing or equivalent.
- Experience in acute care, hospital settings, or home care is required.
- Proficiency in completing comprehensive OASIS assessments.
- Pediatric experience is preferred for this role.
- Must possess a vehicle and valid driver’s license for travel.
- Experience with Homecare Homebase (HCHB) software is a plus.
