Key Responsibilities & Duties

  • Provide visiting home health services, including patient admissions, re-certifications, and discharges.
  • Develop and coordinate Physician Plans of Care with healthcare professionals.
  • Document patient care using a tablet within the required timeframe.
  • Utilize OASIS D1 assessments for admissions, re-certifications, transfers, and discharges.
  • Ensure compliance with Department of Health regulations and agency standards.
  • Support the agency’s mission by delivering culturally competent and high-quality care.
  • Collaborate with the Clinical Manager and healthcare team to achieve patient recovery goals.

Job Requirements

  • Valid Nursing license and proven experience as a registered nurse.
  • Associate of Applied Science (AAS) degree in Nursing or equivalent.
  • Experience in acute care, hospital settings, or home care is required.
  • Proficiency in completing comprehensive OASIS assessments.
  • Pediatric experience is preferred for this role.
  • Must possess a vehicle and valid driver’s license for travel.
  • Experience with Homecare Homebase (HCHB) software is a plus.
