Overview

- Organize and facilitate family meetings to support patient care and decision-making processes.

- Ensure compliance with healthcare regulations and guidelines, including advance directives and protective services processes.
- Participate in the development and evaluation of patient care strategies and interventions.
- Support patients in navigating healthcare systems, including HMOs and IPAs.
Key Responsibilities & Duties
- Conduct comprehensive assessments of patient needs and develop individualized care plans.
- Provide counseling and support to patients experiencing mental health crises or homelessness.

Job Requirements
- Master of Social Work degree required, with entry-level experience in case management.

- Knowledge of healthcare systems, including HMOs, IPAs, and protective services processes.
