Job Description

Overview

  • Provide case management services to patients in a healthcare setting, ensuring comprehensive care coordination and support.
  • Utilize expertise in social work to address patient needs, including mental health, homelessness, and substance abuse concerns.
  • Collaborate with medical teams to develop and implement patient treatment plans effectively.
  • Organize and facilitate family meetings to support patient care and decision-making processes.
  • Coordinate services such as durable medical equipment, skilled nursing facilities, and ambulance transport independently.
  • Ensure compliance with healthcare regulations and guidelines, including advance directives and protective services processes.
  • Participate in the development and evaluation of patient care strategies and interventions.
  • Support patients in navigating healthcare systems, including HMOs and IPAs.

Key Responsibilities & Duties

  • Conduct comprehensive assessments of patient needs and develop individualized care plans.
  • Provide counseling and support to patients experiencing mental health crises or homelessness.
  • Collaborate with interdisciplinary teams to ensure seamless care delivery and coordination.
  • Facilitate family meetings to address patient care concerns and enhance communication.
  • Arrange necessary services, including transportation and medical equipment, for patient care continuity.
  • Educate patients and families on healthcare options, advance directives, and treatment plans.
  • Ensure adherence to healthcare policies and procedures in all case management activities.
  • Participate in ongoing professional development to maintain expertise in social work practices.

Job Requirements

  • Master of Social Work degree required, with entry-level experience in case management.
  • Minimum of 2 years of experience in social work or related field preferred.
  • Proficiency in addressing mental health, homelessness, and substance abuse issues.
  • Ability to independently coordinate services such as DME, SNF, and transportation.
  • Knowledge of healthcare systems, including HMOs, IPAs, and protective services processes.
  • Competence in facilitating family meetings and developing treatment plans collaboratively.
  • Strong organizational skills and ability to manage multiple cases effectively.
