Key Responsibilities & Duties

Job Requirements
- Master's Degree in Social Work from an accredited institution.
- Licensed Clinical Social Worker (LCSW) in the state of Connecticut.
- Minimum of one year experience in hospice or home care preferred.
- Proficiency in end-of-life and palliative care practices.
- Strong communication, group facilitation, and presentation skills.
- Ability to work independently and collaboratively within interdisciplinary teams.
- Knowledge of Connecticut guidelines for licensed hospice social work services.
