Job Description

Overview

  • Lead post-acute care programs and ensure high-quality patient care within a healthcare organization.
  • Oversee daily operations, develop programs, and implement policies to optimize care delivery.
  • Collaborate with interdisciplinary teams to enhance workflows and patient outcomes.
  • Support leadership in managing staff performance, training, and professional development.
  • Monitor compliance with regulatory requirements and organizational policies.
  • Advocate for patient needs and preferences in care planning processes.

Key Responsibilities & Duties

  • Supervise nursing staff and administrative roles within post-acute care programs.
  • Develop and maintain policies and procedures to ensure compliance and quality standards.
  • Monitor and manage post-acute care spending across assigned clinics.
  • Collaborate with leadership to implement strategic plans and enhance team performance.
  • Facilitate interdisciplinary collaboration to ensure comprehensive patient care.
  • Conduct training sessions and professional development programs for nursing staff.
  • Integrate technology solutions to improve communication and documentation processes.
  • Identify opportunities for program expansion and quality improvement initiatives.

Job Requirements

  • Bachelor of Science in Nursing (BSN) required; RN license in the state of employment or compact.
  • Minimum 2 years of supervisory experience in post-acute care settings.
  • Preferred 4 years of experience in post-acute nursing, including home health and rehabilitation.
  • Strong leadership, communication, and organizational skills essential.
  • Experience with value-based care programs or utilization management preferred.
  • Proficiency in process improvement and project management advantageous.
  • Familiarity with EPIC systems and healthcare technology solutions beneficial.
  • Ability to travel occasionally for team coordination and program oversight.
