Key Responsibilities & Duties

• Supervise assigned staff and coordinate client medical and psychosocial needs effectively.
• Act as primary medical case manager, conducting health checks and coordinating with healthcare providers.
• Maintain medication administration records and controlled substance logs accurately.
• Provide health education to clients and advocate for their health preferences and goals.
• Participate in treatment team meetings and case conferences to share findings and client needs.
• Ensure adequate medication supply and follow up on medical treatment plans.
• Deliver emergency first aid and CPR assistance when required.
• Perform related duties as assigned to support the program's objectives.

Job Requirements

• Valid Licensed Practical Nurse (LPN) certification is required.
• Associate of Science (AS) degree in Nursing or related field.
• Minimum of 1 year of experience, with 3 years preferred, in healthcare settings.
• Strong knowledge of mental health and substance abuse treatment protocols.
• Proficiency in maintaining medical records and medication administration logs.
• Ability to provide health education and advocate for client health goals.
• Excellent communication skills for team collaboration and client interaction.
