Key Responsibilities & Duties
- Supervise assigned staff and coordinate client medical and psychosocial needs effectively.
- Act as primary medical case manager, conducting health checks and coordinating with healthcare providers.
- Maintain medication administration records and controlled substance logs accurately.
- Provide health education to clients and advocate for their health preferences and goals.
- Participate in treatment team meetings and case conferences to share findings and client needs.
- Ensure adequate medication supply and follow up on medical treatment plans.
- Deliver emergency first aid and CPR assistance when required.

Job Requirements
- Valid Licensed Practical Nurse (LPN) certification is required.
- Associate of Science (AS) degree in Nursing or related field.
