Overview
- Provide comprehensive medical and psychosocial case management to clients in permanent supportive housing.
- Collaborate with healthcare professionals to optimize client health outcomes and service connections.
- Conduct health and vitals checks, ensuring accurate documentation and medication management.
- Assist clients with practical needs, including service navigation and daily living support.
- Work closely with a multidisciplinary team to advocate for client health preferences and goals.
- Provide health education and emergency first aid/CPR assistance as needed.
- Maintain accurate records, including medication administration logs and controlled substance counts.
- Participate in treatment team meetings and case conferences to share findings and coordinate care.
Key Responsibilities & Duties
- Supervise assigned staff and ensure adherence to medical and psychosocial care standards.
- Link clients to medical and psychiatric services, coordinating with healthcare providers and pharmacies.
- Act as primary medical case manager, conducting health checks and managing client health needs.
- Document client interactions and maintain medication administration records and controlled substance logs.
- Provide health education and advocate for client health preferences and goals.
- Coordinate with nursing leadership and team members to optimize client outcomes.
- Assist clients with practical needs, such as understanding bills or arranging services.
- Deliver emergency first aid/CPR assistance when required.
Job Requirements
- Active LPN license and Associate of Science (AS) degree required.
- Minimum of 1 year experience in a related field; 3 years preferred.
- Strong documentation skills for maintaining medical records and treatment logs.
- Proficiency in coordinating care with healthcare providers and treatment teams.
- Ability to provide health education and advocate for client health goals.
- Experience in managing medication supplies and controlled substance logs.
