Overview
- Serve as a Case Manager in a hybrid role with fieldwork in Queens and remote work opportunities.
- Collaborate with members and families to maintain independent living situations and ensure consistent care.
- Coordinate healthcare services across the continuum, ensuring members receive appropriate support.
- Work closely with interdisciplinary teams, primary care practitioners, and caregivers to optimize care plans.
- Participate in office training sessions and fieldwork to enhance professional expertise.
- Utilize bilingual skills in Cantonese/Mandarin or Spanish to enhance communication with members.

Key Responsibilities & Duties
- Assess and monitor members' healthcare needs to ensure optimal care delivery.
- Authorize covered services and coordinate care plans across multiple healthcare providers.
- Communicate effectively with members, families, and caregivers to address concerns and provide guidance.
- Collaborate with interdisciplinary teams to develop and implement tailored care strategies.
- Conduct field visits in the Queens area to assess member needs and provide support.
- Participate in training sessions to stay updated on best practices and healthcare regulations.
- Maintain accurate documentation of member interactions and care plans.
- Ensure compliance with healthcare policies and procedures.
Job Requirements
- Bachelor of Science in Nursing (BSN) degree required.
- Current NYS RN License and registration mandatory.
- Minimum of 3 years of RN experience, preferably in geriatrics or managed long-term care.
- Bilingual proficiency in Cantonese/Mandarin or Spanish preferred.
- Strong communication and collaboration skills with interdisciplinary teams and caregivers.
- Ability to perform fieldwork two days per week in the Queens area.
- Familiarity with healthcare policies, procedures, and managed care practices.
