Job Description

Overview

  • Provide integrated mental health and substance abuse services to diverse clients, including infants to geriatrics, in a hybrid work environment.
  • Collaborate with a multidisciplinary team to deliver evidence-based therapeutic interventions and crisis management support.
  • Ensure timely access to quality care while adhering to city, state, and federal standards of practice.
  • Utilize electronic health records to maintain accurate documentation of assessments, treatment plans, and client progress.
  • Engage in professional development opportunities and contribute to the achievement of program targets and client goals.
  • Support clients in accessing community resources and navigating social services and criminal justice systems.
  • Participate in new hire orientation and onboarding processes to integrate into the organization effectively.

Key Responsibilities & Duties

  • Deliver direct client care using evidence-based mental health and substance use treatment modalities to optimize client functioning.
  • Conduct assessments, develop person-centered treatment plans, and provide psychotherapy and crisis intervention services.
  • Facilitate group counseling sessions and clinical case management for individuals and families.
  • Collaborate with social service agencies, criminal justice providers, and education systems to coordinate care.
  • Maintain detailed documentation in electronic health records, ensuring compliance with regulatory standards.
  • Meet weekly with supervisors to review program targets, client goals, and quality of care metrics.
  • Participate in case reviews and develop actionable next steps for clients and families.
  • Provide consultation services to child protective specialists and other stakeholders as needed.

Job Requirements

  • Master of Social Work degree with LMSW or LCSW certification required; LMHC or LMFT certifications preferred.
  • Minimum of 2 years of experience in mental health and substance abuse treatment; 3 years preferred.
  • Proficiency in electronic health records and telehealth platforms; Microsoft Office skills essential.
  • Bilingual proficiency in Spanish, Mandarin, Cantonese, or Bengali preferred for effective client communication.
  • Experience with criminal justice systems and military backgrounds considered advantageous.
  • Strong documentation, case note prioritization, and assessment skills required.
  • Ability to work independently, manage multiple tasks, and adhere to timelines effectively.
