Job Description

Overview

  • Provide integrated mental health and substance abuse services to diverse clients, including infants to geriatrics, in a hybrid work environment.
  • Collaborate with multidisciplinary teams to deliver evidence-based therapeutic interventions and crisis management services.
  • Support clients in achieving their goals through person-centered treatment planning and community resource facilitation.
  • Maintain compliance with professional documentation standards using electronic health records.

Key Responsibilities & Duties

  • Deliver direct client care using evidence-based mental health and substance abuse treatment modalities.
  • Provide crisis intervention, therapeutic services, and group counseling to individuals and families.
  • Document client assessments and treatment plans in compliance with regulatory standards.
  • Coordinate care with social services, criminal justice, and education systems to support client needs.
  • Conduct consultations and attend case reviews to develop actionable plans for clients.
  • Meet program targets and quality metrics through effective case management and reporting.
  • Utilize telehealth platforms to deliver remote counseling and support services.
  • Participate in team meetings to discuss client progress and program goals.

Job Requirements

  • Master of Social Work degree with LMSW or LCSW certification required.
  • Minimum of 2 years of experience in mental health and substance abuse treatment.
  • Experience working with clients involved in the criminal justice system preferred.
  • Proficiency in Microsoft Office and electronic health record systems.
  • Bilingual proficiency in Spanish, Mandarin, Cantonese, or Bengali preferred.
  • Strong documentation and case note skills, including prioritization and assessments.
  • Ability to work independently and manage a caseload of approximately 35 clients per week.
  • Military background is a plus.
