Overview

  • Support individuals in developing skills for community living and integration.
  • Foster positive social, emotional, and educational development through tailored interventions.
  • Provide hands-on training to build developmentally appropriate skills in diverse environments.
  • Promote coping strategies and effective functioning within social environments.
  • Enhance self-management of symptoms affecting daily living and routines.
  • Re-establish social skills and achieve developmentally appropriate functioning.
  • Document services provided accurately and in a timely manner.
  • Collaborate with diverse social, cultural, and economic groups effectively.

Key Responsibilities & Duties

  • Provide rehabilitation interventions in home and community environments.
  • Assist individuals in developing interpersonal and social skills.
  • Support individuals in managing symptoms affecting daily living.
  • Facilitate inclusion and integration within community settings.
  • Develop day-to-day competencies for home, school, work, or community environments.
  • Maintain accurate documentation of services provided.
  • Collaborate with team members to ensure effective service delivery.
  • Participate in training and development programs as required.

Job Requirements

  • High School Diploma or GED required; Bachelor's Degree preferred.
  • 1+ years of experience in behavioral health settings.
  • Experience with children’s mental health, addiction, or foster care services.
  • Experience with adults in substance use disorder or serious mental illness services.
  • Certified Psychiatric Rehabilitation Practitioner (CPRP) preferred.
  • Proficiency in Microsoft Office and familiarity with Electronic Health Records (EHR).
  • Strong written, verbal, and listening communication skills.
  • Ability to work effectively with diverse social, cultural, and economic groups.
