Job Description

Overview

  • Support individuals in developing skills to enhance community living and personal functioning.
  • Provide rehabilitation interventions and hands-on training to build appropriate developmental skills.
  • Foster positive social, emotional, and educational development for diverse age groups.
  • Promote coping strategies and effective functioning in social environments.
  • Assist individuals in achieving goals within their community settings.
  • Work in a hybrid environment with a mix of in-person and remote days.
  • Collaborate with a team-oriented approach to deliver impactful services.
  • Engage in a fluid caseload averaging 20 individuals.

Key Responsibilities & Duties

  • Provide rehabilitation interventions in home and community environments.
  • Support development of social skills, interpersonal relationships, and coping mechanisms.
  • Facilitate inclusion and integration within social and educational settings.
  • Document services provided accurately and in a timely manner.
  • Assist individuals in managing symptoms affecting daily living.
  • Promote routines necessary for maintaining home, school, or work environments.
  • Re-establish developmentally appropriate functioning and social etiquette.
  • Collaborate with diverse social, cultural, and economic groups effectively.

Job Requirements

  • High School Diploma or GED required; Bachelor's degree preferred.
  • Minimum 1 year experience in behavioral health settings; 2 years preferred.
  • Experience with children’s mental health, addiction, or foster care services.
  • Experience with adults in substance use disorder or serious mental illness services.
  • Certified Psychiatric Rehabilitation Practitioner (CPRP) certification preferred.
  • Proficiency in Microsoft Office and familiarity with Electronic Health Records (EHR).
  • Trained in Multi-System Family Treatment and Individual Placement and Support Model.
  • Ability to work effectively in hybrid work arrangements.
