Home Health Aide

Overview

• Provide compassionate care to patients in their homes, ensuring comfort and dignity.
• Work in a field-based role visiting 2-3 patients daily in Rockland County.

Key Responsibilities & Duties

• Provide direct patient care including basic first aid and personal assistance.
• Maintain certification through annual continuing education and training.
• Communicate effectively with patients, families, and healthcare staff.
• Document patient information accurately using MS Office and EMR systems.
• Drive to patient locations daily, ensuring timely and reliable service.
• Adhere to hospice philosophy, providing emotional and physical support.
• Collaborate with team members to optimize patient care and outcomes.

Job Requirements

• Certified NYS Home Health Aide with maintained annual training.
• Proficiency in MS Office and electronic medical records systems.
• Access to a reliable vehicle for daily patient visits.
• Strong understanding of hospice philosophy and patient care.
• Effective verbal and written communication skills.
• Ability to maintain confidentiality and professionalism.
