Key Responsibilities & Duties
- Provide direct patient care in accordance with hospice philosophy and care standards.
- Travel daily to patients' homes, ensuring timely and effective service delivery.
- Assist patients with daily living activities, ensuring their comfort and dignity.
- Collaborate with healthcare teams to develop and implement care plans.
- Maintain accurate documentation of patient care and progress in the EMR system.
- Communicate effectively with patients, families, and staff to address needs and concerns.

Job Requirements
- Certified New York State Home Health Aide with annual in-service training completion.
- Bachelor of Science degree preferred; commitment to hospice philosophy required.

- Ability to travel daily with access to a reliable vehicle.
