Overview

  • Provide compassionate care to patients in their homes, ensuring their comfort and well-being.
  • Work in Orange County, visiting 2-3 patients daily to deliver quality care.

Key Responsibilities & Duties

  • Provide direct patient care in accordance with hospice philosophy and care standards.
  • Travel daily to patients' homes, ensuring timely and effective service delivery.
  • Assist patients with daily living activities, ensuring their comfort and dignity.
  • Collaborate with healthcare teams to develop and implement care plans.
  • Maintain accurate documentation of patient care and progress in the EMR system.
  • Communicate effectively with patients, families, and staff to address needs and concerns.
  • Participate in continuing education to maintain certification and enhance skills.
  • Adhere to confidentiality standards regarding patient and family information.

Job Requirements

  • Certified New York State Home Health Aide with annual in-service training completion.
  • Bachelor of Science degree preferred; commitment to hospice philosophy required.
  • Proficiency in basic first aid and MS Office applications; EMR system experience preferred.
  • Ability to travel daily with access to a reliable vehicle.
  • Strong verbal and written communication skills; effective team collaboration.
  • Capacity to relate to diverse populations and maintain confidentiality.
  • Ability to work independently and utilize supervision effectively.
