Overview
- Lead the Care Coordination department, ensuring efficient and cost-effective patient care management.
- Collaborate with various hospital departments to enhance patient care outcomes and financial performance.
- Supervise and coordinate activities of Case Managers and Social Workers within the organization.
- Evaluate operational functions, including staffing, scheduling, and performance management.
- Provide training on patient care data analysis and presentation for organizational goals.
- Ensure adherence to Service Excellence standards across the department.

Key Responsibilities & Duties
- Supervise and manage the activities of Case Managers and Social Workers.
- Collaborate with medical staff and external agencies for seamless patient care coordination.
- Evaluate patient care and financial outcomes impacted by the Care Management system.
- Analyze patient care data and provide insights to achieve organizational goals.
- Ensure operational efficiency through effective staffing and scheduling practices.
- Maintain compliance with healthcare standards and regulations.
- Assist in the professional development of department associates.
- Promote a culture of excellence and continuous improvement within the department.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of 2 years management experience in Case Management or Utilization Review.
- Preferred 5 years of clinical experience in Case Management or Utilization Review.
- Current Registered Nurse license in the State of California.
- Basic Life Support (BLS) or CPR Certification through the American Heart Association.
- Certification in CPUR, CPUM, CCM, or other national certifications preferred.
- Strong leadership and organizational skills to manage departmental operations.
- Ability to analyze and present patient care data effectively.
