Job Description
Overview
- The Intake/Assessment Nurse will assess potential members for program eligibility and explain program details to them and their families.
- Conduct outreach activities within the community and evaluate eligibility and safety of potential members.
- Collaborate with the Intake/Outreach and clinical teams to ensure appropriate service provision to new members.
- Communicate with members, families, referral sources, and physicians for seamless transitions to managed long-term care.
- Perform fieldwork assessments in Brooklyn, ensuring comprehensive member evaluations and eligibility determinations.
- Work Monday through Friday, 9 AM to 5 PM, with no weekend or on-call requirements.

Key Responsibilities & Duties
- Conduct initial intake and assessments for Managed Long-Term Care (MLTC) cases using UAS-NY tools.
- Perform comprehensive evaluations of potential members, determining their eligibility for program inclusion.
- Identify medical, functional, psychosocial, behavioral health, cultural, and linguistic needs of members.
- Engage members and families in decision-making, discussing person-centered service and care plans.
- Complete required assessment forms and documentation accurately and effectively.
- Collaborate with Care Managers to establish continuity of care and address urgent member needs.
- Escalate quality of care issues appropriately to ensure member satisfaction and safety.
- Participate in community outreach activities to promote program awareness and engagement.
Job Requirements
- Current New York State Registered Nurse (RN) license is required.
- Bachelor’s degree in Nursing (BSN) is mandatory; Associate degree in Nursing is acceptable.
- Minimum of one year of experience in an acute care setting is required.
- Proficiency in UAS-NY assessments and Managed Long-Term Care (MLTC) processes is essential.
- Home care and management experience are preferred.
- Proficiency in Microsoft Office applications is required.
- Must possess a valid driver’s license and have access to a car for field travel.
- Strong communication and collaboration skills to engage with members, families, and healthcare professionals.
