Overview

Key Responsibilities & Duties

• Conduct comprehensive assessments of patient needs and develop tailored care plans.
• Coordinate with healthcare providers to ensure seamless delivery of services.

• Educate patients and families on healthcare options and resources.
• Document patient interactions and maintain accurate records in compliance with regulations.

Job Requirements

• Associate of Applied Science (AAS) degree in Nursing or related field.
• Minimum of 2 years of experience in case management or related roles.
• Active RN license in good standing.
• Strong knowledge of healthcare systems and patient care coordination.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a multidisciplinary team environment.
• Proficiency in electronic health record systems and documentation.
