Overview
- Perform initial enrollment and reassessment evaluations for members using the Uniform Assessment System for New York (UAS-NY).
- Collaborate with interdisciplinary teams to develop individualized care plans addressing clinical, psychological, and social needs.
- Ensure accurate documentation and compliance with Department of Health (DOH) regulations and policies.
- Conduct assessments both telephonically and in members' homes, accommodating patient preferences.
- Communicate assessment outcomes to relevant stakeholders, including referral sources and care management teams.
- Provide education on program rules, benefits, and covered services to members and caregivers.
- Support members' enrollment into programs such as CDPAS, completing necessary documentation.
- Maintain productivity standards, completing a minimum of two assessments per day.
Key Responsibilities & Duties
- Conduct functional, psychosocial, and environmental safety assessments for members in compliance with DOH guidelines.
- Determine members' eligibility for services and establish safe care plans.
- Coordinate care plans with members, families, caregivers, and healthcare professionals.
- Complete DOH-approved forms and documentation during face-to-face visits.
- Submit recommendations and documentation to the Care Management team within required timeframes.
- Communicate urgent requests, safety issues, and clinical care needs to the appropriate parties promptly.
- Participate in training sessions and adhere to departmental workflows and policies.
- Ensure all assessments and documentation meet quality standards and regulatory requirements.
Job Requirements
- Associate Degree in Nursing required; Bachelor's Degree in Nursing preferred.
- Active Registered Nurse (RN) license in good standing.
- Minimum of one year of experience in Community Health, Managed Care, or related fields.
- Proficiency in Microsoft Word, Excel, and UAS-NY systems.
- Strong communication skills and ability to explain complex information clearly.
- Bilingual skills in Spanish, Bangla, Hindi, Urdu, Mandarin, or Cantonese preferred.
- Comfortable working in hybrid settings, including telephonic and in-home assessments.
- Ability to manage productivity expectations, completing assigned assessments efficiently.
