Key Responsibilities & Duties

• Conduct comprehensive assessments to identify patient needs and develop appropriate care plans.
• Coordinate with medical professionals to ensure seamless delivery of healthcare services.
• Provide counseling and support to patients and their families during treatment.
• Maintain accurate documentation of patient interactions and case management activities.
• Monitor patient progress and adjust care plans as necessary to meet evolving needs.

Job Requirements

• Master of Social Work degree from an accredited institution is required.
• Minimum of 2 years of experience in case management or related field.
• Licensed Clinical Social Worker (LCSW) certification preferred for enhanced compensation.
• Strong knowledge of healthcare regulations and patient care standards.
• Excellent communication and interpersonal skills for effective patient interaction.
