Overview
- Coordinate telephonic case management for Certified Home Health Agency (CHHA) patients, ensuring high-quality service delivery.
- Develop Physician Plans of Care in collaboration with healthcare professionals to address patient needs effectively.
- Ensure compliance with Department of Health regulations and agency standards in all aspects of care provision.
- Support the agency's mission to deliver exceptional care by maintaining regulatory compliance and quality standards.
- Provide guidance and coordination for an assigned caseload of patients requiring home health services.
- Collaborate with field nurses to ensure seamless communication and service delivery for CHHA patients.
- Maintain accurate documentation and records to support effective case management and regulatory compliance.
- Work on-site during standard business hours to facilitate patient care coordination and agency operations.
Key Responsibilities & Duties
- Manage telephonic communication with CHHA patients to assess needs and coordinate care services.
- Develop and update Physician Plans of Care in collaboration with healthcare providers.
- Ensure all home care services meet Department of Health regulations and agency standards.
- Coordinate with field nurses to ensure quality care delivery and effective communication.
- Maintain detailed patient records and documentation for compliance and quality assurance.
- Provide support and guidance to healthcare professionals involved in patient care.
- Monitor patient progress and adjust care plans as necessary to meet evolving needs.
- Participate in agency meetings and training to stay updated on regulations and standards.
Job Requirements
- Associate of Applied Science (AAS) degree in Nursing or related field.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) certification.
- Minimum of 2 years of experience in Certified Home Health Agency (CHHA) case management.
- Preferred 4 years of experience in home health care coordination.
- Strong knowledge of Department of Health regulations and compliance standards.
- Excellent communication and organizational skills for effective case management.
- Ability to work on-site during standard business hours (Monday to Friday, 9 AM to 5 PM).
- Proficiency in telephonic communication and patient care coordination.
