Key Responsibilities & Duties
- Coordinate and assess home healthcare needs of referred patients, ensuring timely intake and discharge planning.
- Develop comprehensive treatment plans aligned with medical orders, insurance guidelines, and patient requirements.
- Order necessary medical equipment, supplies, and ancillary services, ensuring proper documentation.
- Schedule and assign visits for field staff, ensuring efficient resource utilization.
- Conduct case conferences to ensure effective case management and coordination among clinical personnel.
- Review admission and recertification documentation to ensure care plans meet patient needs.
- Participate in on-call supervisor rotation, addressing urgent patient care needs.
- Demonstrate knowledge of reimbursement practices and state/federal regulations.
Job Requirements
- License to practice as a Registered Nurse; BSN preferred.
- Minimum three years of nursing experience, with two years in home health preferred.
- Proficiency in OASIS and clinical documentation standards.
- Effective verbal and written communication skills.
