Overview

  • Lead and manage clinical operations for a home healthcare agency, ensuring quality patient care and compliance with regulations.
  • Coordinate patient assessments and develop individualized care plans in collaboration with healthcare professionals.
  • Provide guidance and oversight to clinical field staff, ensuring optimal case management and documentation accuracy.
  • Act as a resource for agency inquiries, delivering courteous and professional support to patients and families.
  • Participate in quality improvement initiatives and staff orientation programs to enhance agency performance.
  • Collaborate with managed care personnel to secure necessary authorizations for patient services.
  • Maintain confidentiality and demonstrate cultural sensitivity in all patient interactions.
  • Utilize community resources effectively to meet diverse patient care needs.

Key Responsibilities & Duties

  • Coordinate and assess home healthcare needs of referred patients, ensuring timely intake and discharge planning.
  • Develop comprehensive treatment plans aligned with medical orders, insurance guidelines, and patient requirements.
  • Order necessary medical equipment, supplies, and ancillary services, ensuring proper documentation.
  • Schedule and assign visits for field staff, ensuring efficient resource utilization.
  • Conduct case conferences to ensure effective case management and coordination among clinical personnel.
  • Review admission and recertification documentation to ensure care plans meet patient needs.
  • Participate in on-call supervisor rotation, addressing urgent patient care needs.
  • Demonstrate knowledge of reimbursement practices and state/federal regulations.

Job Requirements

  • License to practice as a Registered Nurse; BSN preferred.
  • Minimum three years of nursing experience, with two years in home health preferred.
  • Proficiency in OASIS and clinical documentation standards.
  • Effective verbal and written communication skills.
  • Strong problem-solving abilities and organizational skills.
  • Commitment to customer relations and quality service.
  • Knowledge of state and federal healthcare regulations.
  • Ability to manage multiple responsibilities and prioritize effectively.
