Job Description

Overview

  • Provide comprehensive nutritional care to participants in an Adult Day Program setting.
  • Collaborate with interdisciplinary teams to ensure optimal dietary plans and implementation.
  • Develop and revise nutritional assessments and care plans as needed.
  • Educate participants on nutrition and dietary practices tailored to their needs.
  • Prepare and distribute meals, snacks, and ingredients for participants.
  • Ensure compliance with dietary prescriptions and physician orders.
  • Support the implementation and revision of therapeutic and cycle menus.
  • Respect cultural and religious dietary preferences of participants.

Key Responsibilities & Duties

  • Conduct initial and periodic nutritional assessments for participants.
  • Participate in interdisciplinary care plan meetings to discuss participant needs.
  • Evaluate and follow up on nutritional care plans to ensure effectiveness.
  • Plan and modify diets based on physician prescriptions and participant requirements.
  • Develop educational materials and conduct in-service training sessions.
  • Monitor and adjust dietary prescriptions to align with physician orders.
  • Assist with audits and performance improvement programs.
  • Collaborate with food and nutrition staff to ensure quality meal preparation and distribution.

Job Requirements

  • Bachelor of Science in Nutrition or related field required.
  • Registered Dietitian certification mandatory.
  • Clinical experience in long-term care preferred.
  • Ability to assess nutritional status and develop individualized care plans.
  • Proficiency in documentation adhering to best practices and regulatory guidelines.
  • Effective communication skills for interdisciplinary collaboration.
  • Knowledge of therapeutic and cycle menu development.
