Overview
- Provide nutritional care and support for elderly participants in an adult day care program.
- Conduct initial and periodic nutritional assessments to ensure optimal health outcomes.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Prepare and distribute meals, ensuring compliance with dietary prescriptions and nutritional requirements.
- Educate clients on nutrition and provide counseling to promote healthy eating habits.
- Assist in menu planning, development, and revision for general and therapeutic diets.
- Maintain documentation that adheres to best practices and regulatory guidelines.
- Respect cultural and religious dietary preferences of participants.
- Participate in quality improvement programs and audits as needed.
Key Responsibilities & Duties
- Conduct nutritional assessments and reassessments for program participants.
- Collaborate with healthcare teams during interdisciplinary care plan meetings.
- Evaluate and monitor nutritional care plans, ensuring continuity of care.
- Develop and deliver nutrition education materials and in-service training.
- Plan and modify diets based on physician prescriptions and participant needs.
- Provide diet counseling to participants and their families as required.

Job Requirements
- Bachelor of Science in Nutrition or related field required.
- Registered Dietitian certification mandatory.
- Clinical experience in long-term care settings preferred.
