Job Description

Overview

  • Provide nutritional care and support for older adults in an Adult Day Program setting.
  • Collaborate with interdisciplinary teams to ensure optimal dietary plans and client health.
  • Prepare, distribute, and monitor meals and snacks tailored to individual needs.
  • Conduct initial and ongoing nutritional assessments to maintain client well-being.
  • Develop and implement cycle menus for therapeutic and general diets.
  • Provide nutrition education and counseling to clients as required.
  • Ensure compliance with cultural and religious dietary preferences.
  • Support quality improvement initiatives and audits within the program.

Key Responsibilities & Duties

  • Perform nutritional assessments and reassessments, updating care plans as needed.
  • Attend interdisciplinary care plan meetings to contribute dietary expertise.
  • Evaluate and monitor dietary prescriptions, ensuring alignment with physician orders.
  • Develop and present nutrition education materials and in-service training sessions.
  • Plan and modify diets based on medical prescriptions and client needs.
  • Provide diet counseling to clients, addressing individual health concerns.
  • Collaborate with nursing and food service teams to ensure dietary compliance.
  • Assist in the development and revision of menus for therapeutic diets.

Job Requirements

  • Bachelor of Science in Nutrition or related field required.
  • Registered Dietitian certification mandatory; clinical experience preferred.
  • Experience in long-term care or similar settings is advantageous.
  • Proficiency in assessing nutritional status and developing individualized care plans.
  • Strong documentation skills adhering to best practices and regulatory guidelines.
  • Effective communication skills for interdisciplinary collaboration.
  • Ability to respect and incorporate cultural and religious dietary preferences.
  • Availability for on-site work during scheduled hours.
