Job Description

Overview

  • Provide nutritional care and guidance for participants in an adult day program.
  • Collaborate with interdisciplinary teams to ensure comprehensive care plans.
  • Plan, calculate, and modify diets based on individual needs and physician orders.
  • Monitor and adjust diet prescriptions to align with health objectives.
  • Conduct initial and quarterly nutritional assessments for participants.
  • Develop and present nutrition education materials and in-service training.
  • Assist with menu development and implementation for therapeutic diets.
  • Respect cultural and religious dietary preferences of participants.
  • Support quality improvement initiatives and audits within the program.

Key Responsibilities & Duties

  • Perform nutritional assessments and reassessments for program participants.
  • Participate in interdisciplinary care plan meetings to discuss participant needs.
  • Provide follow-up care and counseling to ensure continuity of nutritional services.
  • Develop and revise cycle menus for general and therapeutic diets.
  • Monitor compliance with diet prescriptions and make necessary adjustments.
  • Educate participants and staff on nutrition-related topics and best practices.
  • Collaborate with nursing and food service teams for optimal care delivery.
  • Ensure documentation meets regulatory and best practice guidelines.
  • Support program audits and performance improvement initiatives.

Job Requirements

  • Bachelor of Science in Nutrition or related field required.
  • Registered Dietitian certification mandatory.
  • Experience in long-term care or working with elderly populations preferred.
  • Proficiency in developing individualized care plans and nutritional assessments.
  • Strong communication skills for interdisciplinary collaboration and participant counseling.
  • Ability to respect and incorporate cultural and religious dietary preferences.
  • Knowledge of regulatory guidelines and best practices in nutritional care.
  • Competence in menu planning and therapeutic diet implementation.
  • Availability for on-site work during scheduled hours.
