Overview
- Provide nutritional care and guidance for participants in an adult day program.
- Collaborate with interdisciplinary teams to ensure comprehensive care plans.
- Plan, calculate, and modify diets based on individual needs and physician orders.
- Monitor and adjust diet prescriptions to align with health objectives.
- Conduct initial and quarterly nutritional assessments for participants.
- Develop and present nutrition education materials and in-service training.
- Assist with menu development and implementation for therapeutic diets.
- Respect cultural and religious dietary preferences of participants.
- Support quality improvement initiatives and audits within the program.
Key Responsibilities & Duties
- Perform nutritional assessments and reassessments for program participants.
- Participate in interdisciplinary care plan meetings to discuss participant needs.
- Provide follow-up care and counseling to ensure continuity of nutritional services.
- Develop and revise cycle menus for general and therapeutic diets.
- Monitor compliance with diet prescriptions and make necessary adjustments.
- Educate participants and staff on nutrition-related topics and best practices.

Job Requirements
- Bachelor of Science in Nutrition or related field required.
- Registered Dietitian certification mandatory.
- Experience in long-term care or working with elderly populations preferred.
- Proficiency in developing individualized care plans and nutritional assessments.
- Strong communication skills for interdisciplinary collaboration and participant counseling.
