Job Description

Overview

  • Provide clinical nutrition services to older adults in an Adult Day Care program.
  • Ensure nutritional needs are met through assessments, meal planning, and dietary modifications.
  • Collaborate with interdisciplinary teams to develop and implement care plans.
  • Prepare and distribute meals, snacks, and nutritional materials.
  • Educate clients on nutritional practices and provide counseling as needed.
  • Monitor and adjust diet prescriptions in accordance with physician orders.
  • Support quality improvement initiatives and participate in audits.
  • Respect cultural and religious dietary preferences of clients.
  • Contribute to the development and revision of therapeutic and general cycle menus.

Key Responsibilities & Duties

  • Conduct initial and quarterly nutritional assessments for program participants.
  • Participate in interdisciplinary care plan meetings to ensure comprehensive care.
  • Evaluate and provide follow-up on nutritional care plans for continuity.
  • Develop and present nutrition education materials and in-service training.
  • Plan and modify diets based on physician orders and participant needs.
  • Monitor and adjust diet prescriptions to align with medical guidelines.
  • Provide diet counseling to participants to promote healthy eating habits.
  • Assist in quality improvement programs and audits within the community.
  • Ensure documentation meets best practices and regulatory standards.

Job Requirements

  • Bachelor of Science (BS) in Nutrition or related field.
  • Registered Dietitian certification with clinical experience preferred.
  • Experience working with elderly populations in SNF, AL, IL, ADC, or homecare settings.
  • Proficiency in assessing nutritional status and developing individualized care plans.
  • Effective communication skills for interdisciplinary collaboration.
  • Ability to respect cultural and religious dietary preferences.
  • Familiarity with regulatory guidelines and best practices in documentation.
  • Availability for on-site work during scheduled hours.
  • Willingness to undergo training on-site as required.
