Job Description
Overview
- Provide clinical nutrition services to older adults in an Adult Day Care program.
- Ensure nutritional needs are met through assessments, meal planning, and dietary modifications.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Prepare and distribute meals, snacks, and nutritional materials.
- Educate clients on nutritional practices and provide counseling as needed.
- Monitor and adjust diet prescriptions in accordance with physician orders.
- Support quality improvement initiatives and participate in audits.
- Respect cultural and religious dietary preferences of clients.
- Contribute to the development and revision of therapeutic and general cycle menus.
Key Responsibilities & Duties
- Conduct initial and quarterly nutritional assessments for program participants.
- Participate in interdisciplinary care plan meetings to ensure comprehensive care.
- Evaluate and provide follow-up on nutritional care plans for continuity.
- Develop and present nutrition education materials and in-service training.
- Plan and modify diets based on physician orders and participant needs.
- Monitor and adjust diet prescriptions to align with medical guidelines.
- Provide diet counseling to participants to promote healthy eating habits.
- Assist in quality improvement programs and audits within the community.
- Ensure documentation meets best practices and regulatory standards.
Job Requirements
- Bachelor of Science (BS) in Nutrition or related field.
- Registered Dietitian certification with clinical experience preferred.
- Experience working with elderly populations in SNF, AL, IL, ADC, or homecare settings.
- Proficiency in assessing nutritional status and developing individualized care plans.
- Effective communication skills for interdisciplinary collaboration.
- Ability to respect cultural and religious dietary preferences.
- Familiarity with regulatory guidelines and best practices in documentation.
- Availability for on-site work during scheduled hours.
- Willingness to undergo training on-site as required.
