Overview
- Provide clinical nutrition services to older adults in an Adult Day Program setting.
- Plan, prepare, and distribute meals and snacks to program participants.
- Collaborate with interdisciplinary teams to ensure comprehensive care for participants.
- Conduct initial nutritional assessments and quarterly reassessments for participants.
- Develop and implement therapeutic and regular cycle menus tailored to participants' needs.
- Provide diet counseling and nutrition education to participants and caregivers.
- Monitor and modify diet prescriptions in accordance with physician orders.
- Participate in interdisciplinary care plan meetings and contribute to care strategies.
- Support quality improvement initiatives and audits within the community.
Key Responsibilities & Duties

Job Requirements
- Bachelor of Science in Nutrition or related field required.
- Registered Dietitian certification mandatory.
- Minimum of 1 year of clinical experience, preferably in long-term care settings.
- Proficiency in assessing nutritional status and developing individualized care plans.
- Effective communication skills for interdisciplinary collaboration and participant counseling.
- Ability to respect and integrate cultural and religious dietary preferences.
- Knowledge of therapeutic diet planning and implementation.
- Experience with quality improvement initiatives and audits preferred.
