Job Description

Overview

  • Provide clinical nutrition services to older adults in an Adult Day Program setting.
  • Plan, prepare, and distribute meals and snacks to program participants.
  • Collaborate with interdisciplinary teams to ensure comprehensive care for participants.
  • Conduct initial nutritional assessments and quarterly reassessments for participants.
  • Develop and implement therapeutic and regular cycle menus tailored to participants' needs.
  • Provide diet counseling and nutrition education to participants and caregivers.
  • Monitor and modify diet prescriptions in accordance with physician orders.
  • Participate in interdisciplinary care plan meetings and contribute to care strategies.
  • Support quality improvement initiatives and audits within the community.

Key Responsibilities & Duties

  • Conduct comprehensive nutritional assessments and develop individualized care plans.
  • Prepare and distribute meals and snacks, ensuring dietary needs are met.
  • Collaborate with healthcare professionals during interdisciplinary care meetings.
  • Monitor and adjust therapeutic diets based on medical prescriptions and participant needs.
  • Provide nutrition education and counseling tailored to individual participant requirements.
  • Assist in the development and implementation of cycle menus for therapeutic diets.
  • Ensure compliance with cultural and religious dietary preferences of participants.
  • Participate in quality improvement programs and audits to enhance service delivery.
  • Support meal preparation and distribution efforts alongside other team members.

Job Requirements

  • Bachelor of Science in Nutrition or related field required.
  • Registered Dietitian certification mandatory.
  • Minimum of 1 year of clinical experience, preferably in long-term care settings.
  • Proficiency in assessing nutritional status and developing individualized care plans.
  • Effective communication skills for interdisciplinary collaboration and participant counseling.
  • Ability to respect and integrate cultural and religious dietary preferences.
  • Knowledge of therapeutic diet planning and implementation.
  • Experience with quality improvement initiatives and audits preferred.
  • On-site training provided; familiarity with EMR systems advantageous.
