Job Description

Overview

  • Provide nutritional services for participants in an adult day care program, focusing on clinical nutrition and dietary needs.
  • Work part-time, Monday to Friday, from 9 AM to 2 PM, with potential for full-time as the program grows.
  • Collaborate with interdisciplinary teams to ensure comprehensive care for elderly participants with cognitive or physical limitations.
  • Assist in meal preparation, distribution, and ordering to meet dietary requirements.
  • Develop and implement nutrition education materials and in-service training sessions.
  • Maintain compliance with regulatory guidelines and cultural considerations in dietary planning.
  • Support the organization’s mission and vision through dedicated service and care.

Key Responsibilities & Duties

  • Conduct initial and quarterly nutritional assessments for clients, revising care plans as needed.
  • Participate in interdisciplinary care plan meetings to ensure coordinated care.
  • Evaluate and monitor dietary prescriptions, modifying them in accordance with physician orders.
  • Develop and present nutrition education materials and in-service training sessions for staff and clients.
  • Plan, calculate, and modify therapeutic and general cycle menus for participants.
  • Provide diet counseling and follow-up care to ensure continuity of nutritional services.
  • Assist in performance improvement programs and audits within the community.
  • Respect and incorporate cultural and religious dietary preferences in meal planning.

Job Requirements

  • Bachelor of Science in Nutrition or related field required.
  • Registered Dietitian certification mandatory, with clinical experience in long-term care preferred.
  • Proficiency in assessing nutritional status and documenting care plans adhering to best practices.
  • Experience working with elderly populations in settings such as SNF, AL, IL, ADC, or homecare.
  • Ability to develop and revise therapeutic and general cycle menus.
  • Effective communication skills for interdisciplinary collaboration and client counseling.
  • Willingness to work on-site and undergo training as required.
