Overview
- Serve as a Patient Care Liaison in a dynamic healthcare environment, facilitating patient admissions and transitions to nursing home settings.
- Collaborate with healthcare professionals, including physicians, social workers, and case managers, to ensure seamless patient care coordination.
- Engage in marketing and educational activities to promote services to hospitals and community resources.
- Develop and maintain strong relationships with healthcare providers and community organizations.
- Provide effective communication and support to patients and their families during transitions.
- Contribute to social media and marketing initiatives to enhance outreach efforts.
- Participate in planning and organizing community relations and educational programs.
- Ensure compliance with policies and procedures related to nursing facility operations.
Key Responsibilities & Duties
- Visit hospitals daily to establish and maintain relationships with social workers and case managers.
- Act as a liaison between healthcare professionals and agency employees to coordinate safe transitions.
- Develop marketing strategies and manage social media presence to promote services.
- Organize and present ideas and concepts effectively to community groups and agencies.
- Communicate effectively, both orally and in writing, to ensure clarity and understanding.
- Collaborate with local hospitals, physicians’ offices, and assisted living organizations to build networks.
- Plan and execute community relations activities to enhance organizational presence.
- Fulfill additional relevant responsibilities as assigned by management.
Job Requirements
- Associate of Arts (AA) degree or equivalent educational background required.
- Minimum of 1 year of experience as a Patient Care Liaison in a nursing home setting.
- Proven ability to network with local hospitals, physicians’ offices, and assisted living organizations.
- Thorough knowledge of communication principles, mass media, advertising, and community relations.
- Strong organizational skills and ability to present ideas effectively to diverse audiences.
- Excellent oral and written communication skills for effective interaction.
- Ability to plan and execute marketing and social media strategies.
