Overview
- Lead clinical operations for home care and hospice programs, ensuring compliance with Connecticut and federal regulations.
- Provide strategic direction for nursing services, emphasizing quality care and patient-centered approaches.
- Supervise and mentor nursing staff, fostering professional growth and maintaining high standards.
- Develop and implement clinical policies, procedures, and competency assessments for staff.
- Collaborate with interdisciplinary teams to ensure seamless patient care coordination.
- Manage budgets and resource allocation for nursing services effectively.
- Act as liaison with regulatory bodies during inspections and audits.
- Participate in executive leadership to shape organizational strategies and priorities.
Key Responsibilities & Duties
- Provide day-to-day leadership for home care and hospice nursing services.
- Ensure compliance with Connecticut DPH regulations and federal Conditions of Participation.
- Develop and maintain clinical policies, procedures, and orientation curricula.
- Supervise and evaluate nursing leadership and clinical staff performance.
- Oversee clinical quality programs, including performance improvement and utilization review.
- Coordinate patient care transitions with physicians and community providers.
- Maintain workforce credentials and ensure regulatory compliance.
- Manage budgets and support recruitment and retention initiatives.
Job Requirements
- Active Registered Nurse (RN) license in Connecticut.
- Minimum 7 years of clinical nursing experience, with 4 years in home healthcare.
- Bachelor of Science in Nursing (BSN) from an accredited program.
- Certification in Hospice and Palliative Nursing (CHPN) preferred.
- Experience with EMR systems and familiarity with Medicare and Medicaid requirements.
- Demonstrated leadership in regulatory compliance and staff development.
- Excellent interpersonal and documentation skills.
- Proven success in quality improvement and workforce development initiatives.
