Job Description

Overview

  • Provide case management services to support housing stability and reduce social isolation among residents.
  • Assist clients in accessing benefits, entitlements, and programs through counseling and advocacy.
  • Develop and implement care plans tailored to clients' needs and follow up on their progress.
  • Conduct client intakes and screenings to identify presenting problems and appropriate interventions.
  • Maintain accurate and complete case notes and client data using program systems.
  • Prepare required documentation, compile statistics, and generate reports as needed.
  • Participate actively in staff meetings and supervision sessions to exchange information.
  • Provide supportive counseling and arrange translation services for clients when necessary.

Key Responsibilities & Duties

  • Conduct client intakes and screenings to assess social, emotional, and environmental status.
  • Assist clients in applying for and recertifying entitlements and benefits.
  • Provide accurate information on programs and services, and assist in accessing them.
  • Develop and implement individualized care plans for clients, ensuring ongoing contact and support.
  • Maintain complete and accurate case files, including documentation and client data.
  • Compile statistics, prepare reports, and disseminate information as required.
  • Participate in supervision and staff meetings to ensure effective communication and collaboration.
  • Provide supportive counseling and advocate for clients with other agencies.

Job Requirements

  • Bachelor’s Degree required, with one year of related experience or training.
  • Proficiency in Microsoft Office Suite and other relevant technologies.
  • Excellent communication and listening skills, with a focus on respectful client interactions.
  • Ability to multitask, work independently, and maintain attention to detail.
  • Bilingual Spanish skills are a plus but not required.
  • Commitment to the organization’s mission and adherence to policies.
  • Experience in case management and supportive counseling preferred.
  • Strong customer service skills, treating clients with dignity and respect.
