Team Manager – Hospice

Overview

• Lead and mentor a team of nursing staff to ensure high-quality hospice and palliative care services.
• Hybrid work arrangement with three days in-office and two days remote.
• Supervise a team of 6-10 full-time staff and 2-3 fee-for-service staff.
• Collaborate with interdisciplinary teams to achieve patient and family care goals.
• Ensure compliance with documentation policies, state, and federal regulations.
• Participate in community presentations to promote hospice services.
• Support the Associate Director of Patient Services in staff scheduling and operational planning.
• Provide orientation and training at designated locations as required.

Key Responsibilities & Duties

• Mentor nursing staff in pain and symptom management and patient-centered care practices.
• Monitor and evaluate nursing documentation for accuracy and compliance.
• Schedule staff assignments in alignment with organizational and regulatory standards.
• Collaborate with interdisciplinary teams during IDT meetings held in the office.
• Provide leadership and guidance to ensure adherence to hospice program philosophy and goals.
• Assist in community outreach efforts to promote hospice services.
• Ensure staff compliance with training and orientation requirements.
• Support operational planning and implementation in collaboration with management.

Job Requirements

• Bachelor of Science in Nursing (BSN) from an accredited institution.
• Minimum of five years of hospice nursing experience required.
• Supervisory or leadership experience in a healthcare setting preferred.
• New York State RN licensure required.
• National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year of hire.
• Proficiency in Word, Outlook, and Windows software.
• Strong ability to supervise and mentor hospice nursing staff effectively.
• Experience working within an interdisciplinary team environment.
