Overview
- Lead and mentor a team of nursing staff to ensure high-quality hospice and palliative care services.
- Hybrid work arrangement with three days in-office and two days remote.
- Supervise a team of 6-10 full-time staff and 2-3 fee-for-service staff.
- Collaborate with interdisciplinary teams to achieve patient and family care goals.
- Ensure compliance with documentation policies, state, and federal regulations.
- Participate in community presentations to promote hospice services.
- Support the Associate Director of Patient Services in staff scheduling and operational planning.
- Provide orientation and training at designated locations as required.
Key Responsibilities & Duties
- Mentor nursing staff in pain and symptom management and patient-centered care practices.
- Monitor and evaluate nursing documentation for accuracy and compliance.
- Schedule staff assignments in alignment with organizational and regulatory standards.
- Collaborate with interdisciplinary teams during IDT meetings held in the office.
- Provide leadership and guidance to ensure adherence to hospice program philosophy and goals.
- Assist in community outreach efforts to promote hospice services.
- Ensure staff compliance with training and orientation requirements.
- Support operational planning and implementation in collaboration with management.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of five years of hospice nursing experience required.
- Supervisory or leadership experience in a healthcare setting preferred.
- New York State RN licensure required.
- National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year of hire.
- Proficiency in Word, Outlook, and Windows software.
- Strong ability to supervise and mentor hospice nursing staff effectively.
- Experience working within an interdisciplinary team environment.
