Overview

  • Provide comprehensive nursing care to hospice patients, focusing on physical and psychosocial assessments.
  • Develop and implement individualized care plans tailored to patients' end-of-life needs.
  • Collaborate with interdisciplinary teams to ensure holistic patient care.
  • Supervise and evaluate home health aides and personal care workers in patients' homes.
  • Request and manage physician orders for patient care plan adjustments.
  • Conduct fieldwork exclusively within Brooklyn, ensuring high-quality hospice care delivery.
  • Work rotating weekends to provide consistent patient support.
  • Utilize clinical data input systems proficiently to maintain accurate patient records.

Key Responsibilities & Duties

  • Perform thorough physical and psychosocial assessments for hospice patients.
  • Develop nursing care plans based on patient assessments and needs.
  • Supervise and train home health aides and personal care workers.

Job Requirements

  • Associate of Applied Science (AAS) degree in Nursing required.
  • Registered Nurse (RN) certification mandatory.
  • Minimum of 2 years of clinical nursing experience required.
  • Experience in hospice, home care, or acute care preferred.
  • Proficiency in clinical data input systems and electronic health records.
