Key Responsibilities & Duties
- Conduct patient assessments and develop comprehensive care plans in coordination with physicians.
- Provide visiting home health services, including admissions, re-certifications, and discharges.
- Educate patients and families on preventive care and healthy lifestyle practices.
- Document patient care activities using electronic systems within required timeframes.
- Ensure compliance with Department of Health standards and agency protocols.
- Collaborate with interdisciplinary teams to provide holistic patient care.
- Participate in ongoing training and professional development opportunities.
- Maintain effective communication with patients, families, and healthcare professionals.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent qualification.
- Valid nursing license and proven experience as a registered nurse.
- Experience in pediatric care and acute care in hospital or home settings.
- Proficiency in completing comprehensive OASIS assessments.
- Experience using Homecare Homebase (HCHB) is preferred but not required.
- Must possess a vehicle and valid driver's license for travel.
- Strong organizational and communication skills to manage caseload effectively.
