Registered Nurse

Overview

• Provide high-quality nursing care to patients in a home health setting, ensuring their well-being and recovery.
• Educate patients and families on health prevention and healthy habits to promote wellness.
• Collaborate with healthcare professionals to develop and execute individualized care plans.
• Utilize the OASIS D1 assessment for patient admissions, re-certifications, transfers, and discharges.
• Document patient care and progress using provided technology within specified timeframes.
• Ensure compliance with health and safety guidelines and government regulations.
• Participate in orientation and training sessions to enhance professional skills.
• Support the agency's mission to deliver culturally diverse and high-quality care.

Key Responsibilities & Duties

• Conduct patient assessments and develop comprehensive care plans in coordination with physicians.
• Provide visiting home health services, including admissions, re-certifications, and discharges.
• Educate patients and families on preventive care and healthy lifestyle practices.
• Document patient care activities using electronic systems within required timeframes.
• Ensure compliance with Department of Health standards and agency protocols.
• Collaborate with interdisciplinary teams to provide holistic patient care.
• Participate in ongoing training and professional development opportunities.
• Maintain effective communication with patients, families, and healthcare professionals.

Job Requirements

• Associate of Applied Science (AAS) in Nursing or equivalent qualification.
• Valid nursing license and proven experience as a registered nurse.
• Experience in pediatric care and acute care in hospital or home settings.
• Proficiency in completing comprehensive OASIS assessments.
• Experience using Homecare Homebase (HCHB) is preferred but not required.
• Must possess a vehicle and valid driver's license for travel.
• Strong organizational and communication skills to manage caseload effectively.
