Job Description

Overview

  • Provide comprehensive primary care to patients, focusing on holistic health outcomes and value-based care principles.
  • Work on-site during weekdays, ensuring accessible healthcare services without after-hours or weekend commitments.
  • Collaborate with a multidisciplinary team to deliver patient-centered care and achieve organizational goals.
  • Engage in continuous professional development, maintaining board certification and staying updated on evidence-based practices.
  • Manage a patient panel, ensuring quality care and effective communication to build trust and improve health outcomes.
  • Participate in organizational initiatives, including recruitment and training, to support the growth and success of the healthcare center.
  • Utilize technology effectively for documentation and patient management, ensuring efficiency and accuracy in care delivery.

Key Responsibilities & Duties

  • Diagnose, treat, and manage acute and chronic illnesses in older adult patients.
  • Coordinate care with specialists and hospitalists, ensuring comprehensive patient management.
  • Lead a care team to deliver high-quality, patient-centered services and achieve health outcomes.
  • Engage in patient education and health promotion to encourage lifestyle changes and improved health.
  • Document patient interactions and care plans efficiently using electronic health records.
  • Participate in clinical meetings and quality improvement initiatives to enhance care delivery.
  • Supervise and collaborate with Advanced Practice Practitioners, ensuring adherence to care standards.
  • Maintain compliance with regulatory requirements and organizational policies.

Job Requirements

  • Doctor of Medicine (MD) degree with board certification in Internal Medicine, Family Medicine, or Geriatrics.
  • Active state medical license and DEA certification for controlled substances.
  • Basic Life Support (BLS) certification from the AMA or American Red Cross.
  • Minimum of 1 year of experience in primary care; 5 years preferred.
  • Proficiency in electronic health record systems and Microsoft Office Suite.
  • Strong communication and leadership skills to foster teamwork and patient trust.
  • Commitment to continuous professional development and adherence to evidence-based practices.
  • Ability to travel locally for patient care and organizational activities as needed.
