Job Description

Overview

  • Deliver exceptional physical therapy services directly to patients in their homes, ensuring personalized care and effective rehabilitation.
  • Develop and implement individualized therapy plans tailored to specific patient needs and goals.
  • Conduct thorough evaluations to assess patient conditions, progress, and therapy effectiveness.
  • Collaborate with interdisciplinary healthcare teams to ensure coordinated and comprehensive care.
  • Educate patients and caregivers on therapy techniques and home exercise programs for sustained improvement.
  • Maintain detailed and accurate documentation of patient assessments, progress, and care plans.
  • Supervise Physical Therapy Assistants, providing guidance and ensuring adherence to treatment protocols.
  • Ensure compliance with healthcare standards, organizational policies, and regulatory requirements.
  • Participate in discharge planning and recommend appropriate follow-up care strategies.

Key Responsibilities & Duties

  • Perform comprehensive patient evaluations to design effective and measurable therapy plans.
  • Document patient progress, reassessments, and discharge summaries promptly and accurately.
  • Coordinate with physicians and healthcare teams to optimize therapy outcomes.
  • Educate patients and caregivers on therapy practices and home exercise routines.
  • Supervise and mentor Physical Therapy Assistants, ensuring quality care delivery.
  • Ensure therapy services align with prescribed care plans and organizational standards.
  • Participate in discharge planning, providing recommendations for continued care and therapy.
  • Maintain compliance with healthcare regulations and organizational policies.
  • Effectively manage patient inquiries and scheduling to ensure seamless service delivery.

Job Requirements

  • Doctoral degree in Physical Therapy (PT) or equivalent qualification is required.
  • Minimum of 1 year of professional experience in physical therapy; 3 years preferred.
  • Valid Physical Therapist License and AHA Basic Health Care Life Support HCP Certification.
  • Proficiency in using personal computers and relevant software applications.
  • Strong knowledge of rehabilitation therapy practices and patient care standards.
  • Ability to lift, push, and pull a minimum of 50 lbs and stand for extended periods.
  • Excellent communication skills for patient education and team collaboration.
  • Capability to develop and implement individualized treatment plans effectively.
  • Commitment to maintaining accurate and timely documentation of patient care.
