Overview

  • Provide comprehensive primary care services to a diverse patient population in a collaborative healthcare environment.
  • Deliver patient-centered care with a focus on HIV and transgender health, ensuring accessibility and inclusivity.
  • Work within a multidisciplinary team to ensure coordinated and high-quality healthcare delivery.
  • Engage in professional development and quality improvement initiatives to enhance clinical expertise.
  • Maintain accurate and timely documentation using electronic health records (EPIC).
  • Participate in flexible scheduling options, including daytime, evening, and optional Saturday shifts.
  • Contribute to a mission-driven organization dedicated to LGBTQ+ health and wellness advocacy.

Key Responsibilities & Duties

  • Conduct patient assessments, diagnoses, and treatments across various age groups and health conditions.
  • Provide evidence-based care with compassion and respect, ensuring unbiased treatment.
  • Collaborate with healthcare professionals to ensure seamless patient care and effective referrals.
  • Participate in case conferences and quality improvement initiatives to enhance healthcare delivery.
  • Engage in continuing medical education to stay updated on best practices and emerging trends.
  • Document patient care promptly and accurately in electronic health records.
  • Adapt to flexible scheduling, including evening and optional Saturday shifts, to meet patient needs.

Job Requirements

  • Bachelor of Science in Nursing (BSN) or equivalent medical degree (MD/DO).
  • Current New York State Certification/Licensure and DEA Controlled Substances Registration Certificate.
  • Board certification or eligibility in Family Medicine, Internal Medicine, or Infectious Disease.
  • Minimum of 1 year of primary care experience; 3 years preferred.
  • Experience in HIV and transgender care is highly desirable.
  • Proficiency in bilingual communication (Spanish/English) is a plus.
  • Strong collaboration skills and commitment to patient-centered care.
