Overview

- Deliver patient-centered care with a focus on HIV and transgender health, ensuring accessibility and inclusivity.

- Maintain accurate and timely documentation using electronic health records (EPIC).
- Participate in flexible scheduling options, including daytime, evening, and optional Saturday shifts.

Key Responsibilities & Duties
- Conduct patient assessments, diagnoses, and treatments across various age groups and health conditions.
- Provide evidence-based care with compassion and respect, ensuring unbiased treatment.
- Collaborate with healthcare professionals to ensure seamless patient care and effective referrals.
- Participate in case conferences and quality improvement initiatives to enhance healthcare delivery.
- Engage in continuing medical education to stay updated on best practices and emerging trends.
- Document patient care promptly and accurately in electronic health records.
- Adapt to flexible scheduling, including evening and optional Saturday shifts, to meet patient needs.
Job Requirements
- Bachelor of Science in Nursing (BSN) or equivalent medical degree (MD/DO).
- Current New York State Certification/Licensure and DEA Controlled Substances Registration Certificate.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Infectious Disease.
- Minimum of 1 year of primary care experience; 3 years preferred.
- Experience in HIV and transgender care is highly desirable.
- Proficiency in bilingual communication (Spanish/English) is a plus.
- Strong collaboration skills and commitment to patient-centered care.
