Overview
- Provide patient-centered primary care services in a collaborative healthcare environment, specializing in HIV and transgender care.
- Work within a multidisciplinary team to deliver high-quality, respectful care to diverse populations.
- Conduct screenings, examinations, diagnoses, and treatments across the lifespan adhering to evidence-based guidelines.
- Document patient care promptly using electronic health records (EPIC) for seamless coordination.

Key Responsibilities & Duties
- Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.
- Provide respectful, compassionate, and unbiased care consistent with evidence-based guidelines.
- Ensure timely follow-up on patient needs, test results, and referrals.
- Collaborate effectively within a team-based care model for coordinated patient care.
- Participate in quality improvement initiatives and case conferences.
- Engage in ongoing professional development, including continuing medical education.
- Document all care promptly in the electronic health record (EPIC).
- Provide primary care services with a focus on HIV and transgender care.
Job Requirements
- Bachelor of Science in Nursing (BSN) or equivalent medical degree (MD/DO).
- New York State Certification/Licensure (MD, DO, or NP) and current NYS Registration.
- DEA Controlled Substances Registration Certificate and current CPR Certification.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Infectious Disease.
- Minimum of 1 year of experience, with 2-3 years preferred in primary care.
- Experience in HIV and transgender care is highly desirable.
- Strong collaboration skills and commitment to accessible, patient-centered care.
- Bilingual proficiency (Spanish/English) is a plus.
